PMH retains its right of sole discretion in determining a patient’s ability to pay. All patients presenting for emergency services will be treated regardless of their ability to pay. For emergent services, applications to the Financial Assistance Program will be completed, received, and evaluated retrospectively and will not delay patients from receiving care.
To request your 1095-C form for you records, please contact HR at kbrown@pmhwv.org or 304-799-7400 ext 1023. PMH will provide the form within 30 days of the request, or by January 31st of the year following the calendar year requested, whichever is later.
Kari Cooper, PharmD., is a 2009 graduate of the WVU School of Pharmacy. She has been the Pharmacy Director for PMH since 2013 and was part of the development team that worked to bring PMH Pharmacy to life. Kari currently resides in Marlinton with her husband and children.
